Manufacturer License Agreement Schedule I

Schedule I must be completed by all licensees to provide the following information:

(i) All facilities at which the licensee’s certified products are manufactured and/or labeled.

(ii) Whether those facilities are operated by the licensee, a subsidiary, an affiliate, or a third-party.

(iii) Whether any of the licensee’s certified products are sold or distributed under a brand, a name, or model name on the temporary label other than the licensee’s name, brand, or model name as listed in the CPD.

We need to add instructions here once we decide how it will work. Specifically, do we want to allow them to save and come back? We should also tell them what they need to have handy to fill this out.
